Patient: 74-year-old female. Diagnosis: type II diabetes, end stage renal failure (on dialysis 3 x week), right leg amputation, atrial fib, bradycardia with pacemaker, stage III decubitus ulcers, hypothyroidism, depression, peripheral vascular disease and history of small bowel obstruction. Patient receives 23 hours per day of home health nursing. Patient is assisted with all activities of daily living. Bed/chair bound, transfers with a hoyer lift. Prior to being placed on the Comfortex Rest-Q mattress, the patient was using a hospital mattress with an air overlay.

Pain Assesssment:
Patient states to nursing that her comfort level has improved after being placed on the Rest-Q mattress. Supporting nursing notes and health assessment review show pain level to be scored as a 1 on a scale of 1-5.
Conclusion:
Patient was at very high risk for continual skin breakdown due to diagnosis and immobility. She had continued to show improvement with healing and had no further skin breakdown. Comfort level increased when placed on the Rest-Q mattress.
